American Academy of Pediatrics Mental Health Toolkit

The American Academy of Pediatrics has developed a Mental Health Toolkit as a resource to assist physicians in the management of patients with behavioral health concerns. Addressing Mental Health Concerns in Primary Care: A Clinicians Toolkit brings together tools and resources in a quick access CD-ROM. The toolkit includes screening assessments, care plans, documentation and referral tools, parent handouts, community resource guides, etc. The toolkit is divided into 5 sections based on the chronic care principles:

  • Community Resources
  • Health Care Financing
  • Support for Children and Families Clinical Information Systems/Delivery System Redesign
  • Decision Support for Clinicians
